Elementary School Admissions

Application Process

  • Applying to Pre-K

    For applicants entering Pre-K, the following must be uploaded to the checklist on your My St. Mary's portal by February 1st for priority consideration. An applicants checklist is generated shortly after an Inquiry or Application is submitted.
    *Please note - an applicant must turn 4 years old on or before September 1st to be considered for our Pre-K Program.


    Application
    Submission of St. Mary's Elementary School's online application which can be found on ST. MARY'S SCHOOL'S WEBSITE.
    Birth Certificate
    A copy of your child's birth certificate must be uploaded to the checklist generated on their My St. Mary's portal.
    Immunization Records
    A copy of your child's shot records must be uploaded to the checklist generated on their My St. Mary's portal.
    Baptism Certificate (if applicable)
    A copy of your child's Catholic Baptism certificate, if applicable, must be uploaded to the checklist generated on their My St. Mary's portal.
    Parishioner Form (if applicable)
    A Parishioner Form must be filled out by you, sent to St. Mary's Parish Office (or your current Catholic Parish) for them to sign and officially stamp, if applicable.

    Early Learning Inventory
    An Early Learning Form must be filled out by your child's current Pre-K3 Teacher or Daycare provider. These confidential documents must be sent from the teacher directly to St. Mary's Admissions Office.
  • Applying to Kindergarten

    For applicants entering Kindergarten, the following must be uploaded to the checklist on your My St. Mary's portal by February 1st for priority consideration. An applicants checklist is generated shortly after an Inquiry or Application is submitted.
    *Please note - an applicant must turn 5 years old on or before September 1st to be considered for Kindergarten at St. Mary's School.


    Application

    2 Teacher Recommendations
    Two Teacher Recommendations must be filled out by two of your child's current teachers. These confidential documents must be sent from the teacher directly to St. Mary's Admissions Office.
    School Records
    A Release of Records form must be signed by a parent or guardian of the applicant and turned in to their current school. This gives permission for the school to send us a copy of their records including report cards, learning plans, attendance, etc.

  • Applying to Grades 6 through 8

    For applicants entering grades 6 through 8, the following must be uploaded to the checklist on your My St. Mary's portal by February 1st for priority consideration. An applicants checklist is generated shortly after an Inquiry or Application is submitted.

November 8: Open House for St. Mary's Elementary School (Pre-K through Grade 8)

January 15: Pre-K Application Deadline    
All completed Pre-K Applications plus the required documentation are due by January 15th for priority consideration. 


February 1
All Pre-K letters of acceptance or wait pool are mailed on February 1st. Once a grade-level is full, a wait pool is established.     


February 1: Kindergarten through Grade 8 Application Deadline     
All completed Kindergarten through Grade 8 Applications plus required documentation are due by February 1st for priority consideration. 


March 1          
All Kindergarten through Grade 8 letters of acceptance or wait pool are mailed on March 1st. Once a grade-level is full, a wait pool is established for each grade level.

Pre-K through Grade 8 Admissions Forms

  • Parishioner Status Form
    Parent must fill out and sign the top portion of this form and then give it to St. Mary's Parish Office (or your registered Catholic Parish). They will sign and stamp it with their official seal. Once complete, you may upload a copy to your My St. Mary's portal. If this is not applicable to your family, please let the Admissions Office know and we will waive the form on your checklist.








  • Early Learning Inventory (Applicants entering Pre-K, Kindergarten, and Grade 1)
    The Early Learning Inventory is for students applying to Pre-K, Kindergarten, and Grade 1. Please send this form to the applicants current Lead Teacher. They are asked to return it directly to St. Mary's Admissions Office as these are confidential documents (the form is waived if a parent receives it). You will see when this step is complete by following the application checklist on your child's My St. Mary's portal.




  • Teacher Recommendation Form (Applicants entering Grades 2 through 8)
    For those applying to Grades 2 through 8, we require two teacher recommendations. Please send this form to two current teachers of main subject areas (preferably Math & Language Arts). If applicant has one teacher for all subject areas, a school councilor is a wonderful option. The two teachers are asked to return their completed form directly to St. Mary's Admissions Office as these are confidential documents (the form is waived if a parent receives it). You will see when these two steps are complete by following the applicant's checklist on your child's My St. Mary's portal.
  • Release of Records
    A parent or guardian must sign this form and submit it to their child's current school office. This gives the current school permission to send school records to St. Mary's. The records sent should consist of report cards, learning plans, attendance, and any other information we may use during the admissions process. You will see when they have been received by following your child's checklist on their My St. Mary's portal.
